If you are coming from the hospital.
- Your hospital social worker will assist you with transportation to Clovernook.
- Your physician will complete transfer forms to send with you.
- To assist with infection control guidelines, do not bring disposable hospital items to Clovernook.
If you are coming from home
- Your physician needs to complete admission forms prior to admission, our admission
- counselor will be working with you.
- If you need assistance with transportation our admission counselor can advise you.
Reminders
- Arrange with us to initiate telephone service if desired. $1.00/day or $20.00/month for local service. Long Distance charges are $ .13/minute.
- Notify friends and church of new living location.
- Bring enough clothing for one week. A clothing hamper will be supplied if needed on admission.
- Special touches can be added to your room. pictures, photo albums, TV or other favorites.
Prior to/or at the time of your admission, Clovernook needs these questions and items to be answered
- Birthday and religion.
- Who to call in case of need, first and second.
- Medicare, social security number and supplementary insurance cards availability to be photo copied for file.
- Sponsor's name for billing purposes.
- Funeral home (this is a state regulation that needs to be on file).
- Laundry to be done by family or facility.
