Clovernook Health Care Pavilion provides a wide range of nursing care services.
We are Medicaid and Medicare certified, providing Skilled, Intermediate and Custodial Nursing Care; additionally, we offer specialized care to meet the needs of our residents including hospice care.
We create a comprehensive care program for each resident that determines what services are needed to maintain maximum comfort and produce maximum benefits. All of our rehabilitational therapies are available to our nursing center residents.
Skilled Care, as ordered by your physician and coordinated by a Registered Nurse, is available 24 hours a day, seven days a week. Intermediate care as administered by Licensed Practical Nurses and Certified Nursing Assistants is also an around-the-clock function at Clovernook. Specialized Care in a secure environment is formulated by our team of professionals and is available for our residents with manageable dementia disorders. Additionally, we provide hospice care, pain management, wound care, cardiac rehabilitation, and other specialized services.
Social workers are available to counsel residents and their families. Our dietitian creates healthful menus for all residents and specific menus for residents with special diets. Social and recreational programs are planned to include all residents - regardless of limitations.
