PRIVATE PAYING RESIDENTS
The daily rate for Intermediate Care of $128.00 for a Semi-private accommodation or $156.00 for a private room or for Skilled Nursing Care of $380.00 Semi-private or $405.00 for a private room Is inclusive of Nursing Services. There is a $100.00 admission fee for new residents. An additional charge by Clovernook for medical supplies and equipment, provided pursuant to Physician's orders, will be billed monthly. Physician services will be billed directly by the resident's personal physician. Charges for the first thirty days are payable upon admission, and will be billed monthly in advance, thereafter. MEDICARE Clovernook is certified as a Skilled Nursing Facility and has an area specifically for Skilled Medicare patients. When the Resident/Patient meets the Medicare eligibility requirements, payment for care at Clovernook, constituting skilled nursing service, can be made not to exceed 100 days in a benefit period. All charges for covered services are paid for the initial 20 days in a semi-private accommodation. If additional skilled care is eligible, Medicare pays all charges, except $96.00 per day from the 21st to 100th day. This charge is the responsibility of the resident and can be submitted to the resident's private insurance carrier with a Medicare Explanation of Benefits. MEDICAID Clovernook is certified by the Ohio Department of Health to participate in the Medicaid program administered by the Ohio Department of Human Services. If an applicant is found to be medically in need of nursing care and meets all the financial eligibility factors as determined by the Ohio Department of Human Services, Clovernook is paid by the Department the difference between income received by the resident in a given month, less $40.00 which is allowed to be retained for the resident's personal needs, and the rate established between Clovernook and the Ohio Department of Human Services.
